Bring your dog Agreement and Release

MEDICAL INFORMATION:

The owner of the dog agrees to maintain copies of all pertinent pet medical information on premises and agrees to the following minimum requirements.

  • All vaccinations, including bordetellosis, are complete and current.
  • Parasite control is being done on a routine basis and the animal is free of ticks and fleas.
  • The dog has no recent history (6 months) of infection or ringworm.

 

The owner of the dog agrees to observe the following guidelines. Authorization is granted at the sole discretion of Mt Hood Meadows and can be revoked at any time.

  • Owner will be solely responsible for all clean-up inside and outside the building.
  • Owner will immediately clean up any accidents or messes using appropriate clean-up and disinfectant measures.
  • Owner will not bring a dog to work that has not been fully house trained.
  • Owner will not bring a dog to work that is ill or behaving abnormally and will remove the dog from the premises if it becomes ill while on Company property.

 

Mt Hood Meadows Bring Your Dog Authorization and RELEASE

  • Owner will not bring a dog to work that has bitten or is aggressive, destructive, or excessively vocal.
  • Owner must control the dog at all times using a leash outdoors and some form of appropriate containment and/or vocal commands in their work area (appropriate containment may vary in different work areas).
  • Owner has sole responsibility for care of the dog while on Company property.
  • Owner must keep a supply of plastic bags with ties at all times and is responsible for the appropriate disposal of dog waste.
  • No dogs participating in this program will be allowed in the following areas: food preparation areas, serving and dining areas, restrooms, medical and patient care facilities, shuttles, mechanical rooms, construction or repair areas, kitchen areas, and any such areas as designated by the Department Director or Designee.

 

WAIVER OF LIABILITY

In consideration of being permitted to bring ones dog to the work place, the dog owner does hereby unconditionally releases, indemnifies, waives, discharges and agrees to hold harmless Mt Hood Meadows Oreg., LLC (“Meadows”) and its owners, partners, employees, directors, agents, affiliates and related entities from any loss, damage, liability and expense, including court costs and attorney fees, that may be incurred as a result of injuries, including death to persons or dogs, or damage to property, directly or indirectly associated with the owner bringing his/her dog to the work place. This release includes claims and liabilities arising from any cause whatsoever, including, but not limited to negligence on the part of Meadows.


BY SIGNING THIS AUTHORIZATION AND RELEASE, THE DOG OWNER EXPRESSLY ACKNOWLEDGES AND REPRESENTS that he/she has carefully read the foregoing terms and conditions, understands the contents thereof and signs voluntarily; he/she is at least eighteen (18) years of age and fully competent; and executes this Authorization and Release intending that he/she, his/her spouse and family members, and his/her heirs, assigns and personal representatives if deceased, be legally bound by same.

OVERVIEW: As part of our Wellness Program at Mt Hood Meadows we have created a program that will allow responsible dog owners who qualify, the opportunity to bring their dog to work for the purpose of promoting employee morale, job satisfaction, and creating a pleasant work environment. Numerous studies have shown that the presence of well-behaved dogs under the watchful eyes of their owners can reduce stress, contribute to a fun work environment, promote movement/activity, increase productivity and creativity and decrease absenteeism. A study done by the CDC also found that pets triggered workplace interactions that would not normally take place thereby improving co-worker relationships and teamwork. This program has been developed with the understanding that not everyone appreciates the presence of dogs and timely communication is encouraged should any issues arise. While Mt Hood Meadows supports the positive benefits that a socialized and well behaved dog may provide to the work environment, we respect the interests of all employees. Employees who have allergies and/or fears will not be subjected to situations within their immediate work environment that exacerbates these conditions. Employees with allergies and/or other conditions that might be adversely affected by exposure to dogs should notify their Director/VP or designee of said conditions. This information will be considered prior to granting approval for bringing a dog to the workplace. In some instances where exposure cannot be prevented, the dog owner may not be allowed to bring their dog to work. This policy may be adopted or revoked at any time at the sole discretion of the responsible Director. In order to offer this privilege, all employees** are required to observe some listed rules.

A Review Board consisting of 3-5 people will handle complaints along with other issues and convene to review the
policy.
